This is what I meant by "the code" because it has text and the link with your IMDb name ID number. Use this for all of your credits just make the appropriate changes to the title and publisher.

If there are co-writers, use this one below. Just add a comma after your entry below and either type their name in or, you can look them up on IMDb and get their IMDb name ID number. You'll find it in the web address (URL) listed with nmXXXXXX, similar to yours.

If there is no publisher (besides you being self published) on the credits, you can remove the "Published by." If there is a publisher, it should be the publisher at the time of the placement.
