The worst thing I ever heard musically was so bad I was actually turning red with embarrassment for him even though it was something I saw on television. Garth Brooks a few years ago sang for the Grammy's. I don't think it was his fault but apparently he had no monitoring in his ear and he sang the entire song so far out of key that I think it was actually in another key- and even then, he was clearly fishing for the notes. There was no way to describe it unless you compare it to listening to some one who sings really loud and out of tune to a song on their IPod when they have no idea someone is listening.
