I used to do a combination of lyrical phrase, then think up a melody with a chord progression.
Now I write the lyrics first and let the lyrics lead me into a melody. I marry them together, yet each can stand on it's own. During the song writing process, I adapt one or the other if necessary. I notate as I go, which makes it easy to find glitches in the lyric's meter and helps me avoid the pitfall of subconsciously altering the melody to fit a new verse. I add the harmony or chords after I have found the melody.
I have only a few successful songs that I created working from chord progression inspired melodies as opposed to quite a few working from the lyrics first. Learning to notate my music made a big difference in my song writing. I tend to be very visual and I can see inconsistencies right away.
